Please include information regarding the school name, Coach’s contact information, and school address in the message portion of the form.
IMPORTANT NOTICE: Do not fill out this form if your school participated in NJFPS in a previous year. School information carries over from year to year. If you are a new coach for an existing school and would like to participate in NJFPS again this year, please send an e-mail to our Executive Director.
